Please fill out all areas of the form. Under the 'Family Life' section, please list details of your family, such as who will be the main caregiver of the dog. This is also a great place to give details about your other pets, if you have a farm or large amount of land for the dog to run around, etc.

 

Please note that if you live in an Apartment or a Rental House, a signed letter from your landlord will be required prior to adoption. This letter must state that the specific dog your are adopting is allowed to live with you in the Apartment or House.

 

If the dog that you are Adopting is not spayed or neutered, you will be required to provide us with a pre-paid spay/neuter certificate from your vet.  Both of the above documents will be verified by us prior to adoption.

 

We believe that the most important factors to adopting out the dogs under our care is a safe and loving environment for the dog.  Please be very honest with us when filling out the adoption application, we do verify all information provided.  In some cases, we will be willing to work with you to get you qualified to adopt, however if you lie, we will not adopt a dog out to you, ever.
